Services May Be Available for Youth Aged Out of Foster Care
Youth ages 18-22 who have aged out of the foster care system may also be able to access Youth in Transition (YIT) Closed Case Services through our Resource for Independent Skills Enhancement (RISE) Program. If eligible, youth may access these services until their 23rd birthday. Eligibility must be determined for each youth regarding which services they are eligible to receive. Services vary by participant and may include assistance with transportation, housing, educational supports and much more! TYCCA only services youth with cases managed by Wayne County MDHHS.

Program Overview
Through the RISE Program, youth can access their YIT closed case services, receive FREE job training to earn a credential, life skills training, employment placement assistance and case management support.
Upon verification of eligibility, youth may be able to access YIT closed case services until their 23rd birthday. PLEASE NOTE: youth may be eligible for some or all of the following YIT services.
If youth are eligible to receive services, the list below is just a few of the vital documents youth will need to submit. Additional documents may be requested. If youth are unable to provide these documents, our team may be able to assist youth with these documents. Please note, youth do not need these documents to get started. Steps to Getting Your YIT Closed Case Services
All eligible participants must complete the following steps to access their YIT services. For the safety of our youth and team members, most services will be provided virtually or over the phone. In-person services are limited, but may be scheduled as needed. Again, our team may be able to assist you with securing your vital documents.
Step 1: Complete & Submit the Referral Form
Step 2: Attend the Information & Enrollment Session
Step 3: Complete Intake to meet with a Case Manager and assess youth's needs
Step 4: Youth Submits Vital Documents needed to complete enrollment and prepare YIT requests
Step 5: Case Manager Submits YIT Requests on youth's behalf to MDHHS for processing
Step 6: Youth Participates in required life skills workshops, job training and receives employment placement assistance along with monthly coaching while waiting to receive YIT requests

To learn general information about YIT services, the enrollment process or how to refer a youth for services, please attend one of our monthly Virtual YIT Information Sessions. Youth, social workers, case managers, community and private youth serving agencies are welcome to attend!
Each month, dates will be posted to this webpage along with information on how to access these sessions via Zoom. PLEASE NOTE: these sessions are strictly for informational purposes only and are not required for a youth to attend in order to receive YIT services.
